Parent/Guardian Permission & Liability Waiver
I give permission for my child to participate in the Youth Legislature of Louisiana session. I understand and agree to the following:
  • I waive all claims against Youth Legislature of Louisiana and its representatives in the event of an accident or injury.
  • In case of illness or emergency, I authorize Youth Legislature of Louisiana to seek medical attention for my child at my expense.
  • I authorize the use of photos or video taken of my child during the session for Youth Legislature of Louisiana.
  • I understand that all registration fees are transferable but non-refundable.
  • I have read and agree to support and enforce the Professional Dress Code below. Students not adhering to the dress code will not be allowed to participate in any role and must either be brought appropriate attire or be picked up by a parent.
BOYS: Dress shirt with collar and a tie (bow ties acceptable); suit/sport coat optional. Dress or khaki pants — no jeans or T-shirts. Dress shoes only, no tennis shoes.

GIRLS: Pants only (khaki acceptable) — no jeans, leggings, or T-shirts. Professional blouse/shirt required. Skirts/dresses permitted only for religious exceptions, below the knee. Comfortable dress shoes only — no tennis shoes, flip-flops, or slippers. No tight, revealing, or midriff-showing clothing; shoulders covered.
